Creating Reports on Questionnaire Data

You can now build detailed Reports using questionnaire data from Assessments. These reports let you break down results by Assessment Types, Categories, and individual Questions. This supports internal reporting, audit preparation, and regulatory responses.

This guide explains how to create these reports, outlines the available options, and provides instructions on configuring filters and aggregation.

Adding a Questionnaire Report to a Dashboard

  1. Open the Dashboard where you want the Report.
  2. Click Show Settings.
  3. In the sidebar, under Components, drag the Report option onto the canvas.
  4. Once dropped, the right-hand panel will open with configuration settings.

Configuring the Report

Step 1: Select the Entity

Choose the Entity holding the questionnaire data, e.g. Third Party Assessments.

Step 2: Select the Chart Type

All standard chart types are available, except for the Risk Matrix.

Step 3: Select the Data Source

To report on questionnaires, select Question Sets from the list. You can then choose to report on one of the following options (single-select):

Question Set LevelOptionDescription
Assessment TypeAssessment Type Name of the Assessment Type
Assessment Type ScoreAggregated score for the Assessment Type
Category Category Value Name of Question Category
Category ScoreAggregated score for the Category
QuestionQuestion Value Actual answer or response
Question ScoreScore assigned to that response

These can be used in either axis, depending on the chart type.

Aggregation

When using Scores (Assessment Type Score, Category Score, Question Score), you can apply an aggregation method:

  • Count

  • Sum

  • Average

  • Min

  • Max

For example, choosing “Average” with Category Value on the vertical axis and Category Score on the horizontal axis gives the average score per category.

Filtering

Use filters to refine the information being reported.

You can filter by:

  • Assessment Type

  • Question Category

  • Question
