The Comments and Attachments on assessments helps you collaborate more effectively with your team and external third parties. This feature can be found on all records with a questionnaire component.
Adding and Replying to Comments
You can use comments to ask follow-up questions, clarify requirements, or respond to feedback—all within the context of a specific question.
How to Add a Comment:

Navigate to the question which requires further commentary and click the Comments link within the question box.

Click into the comment text box that says “Add a comment…”.

Type your message.

Click Add Comment to share your comment.
Your comment will be visible to other users with access to the assessment, and they can respond with their own comments.
Adding Attachments
Sometimes, a written answer isn’t enough. You can upload supporting documents like PDFs, spreadsheets, screenshots, or any relevant files directly to the question.
How to Upload Attachments:
- Navigate to the question which requires further commentary and click the Attachments link within the question box.

Click the Attach File button.

Select one or more files from your device.

Once uploaded, the files will be listed within the right panel on the question for everyone to view and also display a comment with when and who uploaded the files.
Editing or Deleting Comments and Files

You can edit or delete your own comments using the three vertical dots on a comment.

To delete an attachment from a question, click the 'X' icon against the attachment within the list.
